How to make a referral

How to refer to Katharine House

We accept referrals for patients with any advanced life limiting illness from the following surgeries:

  • Banbury Cross Health Centre
  • Bloxham Surgery
  • Bourne End Surgery, Hook Norton
  • Brackley Medical Centre
  • Byfield Surgery
  • Charlbury Medical Practice
  • Chipping Norton Health Centre
  • Cropredy Surgery
  • Deddington Health Centre
  • Hightown Surgery, Banbury
  • Shenington Surgery
  • Sibford Surgery
  • Springfield Surgery, Brackley
  • Windrush Surgery, Banbury
  • Woodlands Health Centre, Banbury
  • Wychwood Surgery, Shipton Under Wychwood

Referrals can be made by:

  • GPs, community matrons, district nurses
  • Hospital teams
  • Allied health and social care professionals
  • The patient or family

Please note that in all cases there needs to be agreement from the GP before any direct contact with the patient takes place.

Community nurse team referrals

Early referral to our community clinical nurse specialist (CNS) team is preferable, even if it leads to discharge for a period of time. Referral forms should be used where possible. It's possible to contact the community team triage nurse to discuss the specialist palliative care needs of the patient and family, but a completed referral form will still be required. Please contact the triage nurse via reception: 01295 811 866 (available 9am-4.30pm, seven days a week).

Inpatient referrals

Inpatient admissions will also be considered from the surgeries indicated, as well as any other part of Oxfordshire. Consideration will also be given to patients from outside the area whose family are registered with one of these GP practices. Please refer by ringing the medical team on 01295 811 866.

Medical outpatients

The medical team are able to see patients on an outpatient basis as required. These patients are usually known to other areas of the service, but referrals may be made directly by letter to the Medical Director.

Lymphoedema referrals

Please refer by letter to the Lymphoedema Practitioner.

Bereavement Services referrals

Living Well referrals

Please see our Living Well referral page, which contains a downloadable referral form.

Hospital Palliative Care Team (Horton Hospital)

Referrals are accepted with the consent of the admitting consultant. The hospital palliative care team can be contacted on 01295 224 195.
